9/20/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. QMHPA informed Ms. Broadnax that this session was going to focus on her mental health. Before QMHPA began the session, QMHPA allowed Ms. Broadnax the opportunity to discuss with QMHPA any current situations that she felt needed to be addressed. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. Intervention: QMHPA engaged Ms. Broadnax in a discussion surrounding her ADLâs and IDALâs. QMHPA engaged Ms. Broadnax in a skill building activity. QMHPA explained to Ms. Broadnax that the purpose of this activity was to assist Ms. Broadnax with appropriate budgeting techniques and how proper budgeting increase independency. QMHPA implemented Ms. Broadnax keep up with a chart for her funds and write down daily needs and wants. QMHPA explained to Ms. Broadnax about being able to complete daily living skills on her own to promote independency. QMHPA assisted Ms. Broadnax with creating a calendar and keeping track of bathing, wearing clean clothes, and completing daily chores. Response: Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Broadnax sated "I know" when budgeting techniques were explained. Ms. Broadnax said she will work on completing a budgeting chart. Ms. Broadnax said she does not have enough money to save and will spend her money on unnecessary items. Ms. Broadnax admitted to not washing her clothes and that's why she had on the same outfit for about two days. Ms. Broadnax explained to QMHPA at times she doesnât feel like changing her clothes because she is depressed or she doesnât feel like moving. Ms. Broadnax discussed with counselor that she will write down things in her calendar when prompted but will not complete them on her own. Plan: QMHPA will assist the client in identifying and attaining adequate sources of income; develop a budget with the client that is based on resources and needs (1-3x weekly).
